SELF DECLARATION FORMS FOR INSURANCE POLICIES
It has come to our attention that some insurance companies are asking insurance policy applicants to fill in self-declaration forms that require clinical information such as blood pressure readings, hypertension complications, Chest X-ray, ECG results, etc.
Please be advised that should your patients request you to provide the clinical information that these self-declaration forms require, or to fill up the forms on their behalf, you can charge an appropriate fee for rendering these services.
You may wish to retain a copy of this note for future reference.
